 I, _______________________________________ (print name), have read New Millenium Limousine's Cancellation Policy and acknowledge that I am 18 years of age or older. I understand that I am responsible to pay for this contract in full 1 week before the day of service.
   Cancellation Policy: New Millenium Limousine Service is not responsible for any cancellation or delay due to extraordinary traveling conditions, such as mechanical problems. In the event of mechanichal problems, we reserve the right to substitute a similar vehicle of equal value or refund the difference. Sun roof will stay closed while limo is moving. Clients will be held liable for any vandalism, negligence or drunkness. All balances must be paid 1 week prior to event.
     All deposits made towards your limousine reservation(s) are non-refundable. However, if you need to cancel your reservation for any reason, you will be able to use your deposit towards any future booking within one year of the original booking date. For airport and point to point transfers, all "no shows" are non-refundable.
Note For Proms: State Alcohol & Beverage Control prohibits the sale or consumption of alcoholic beverages by anyone under 21 years of age. No DRUGS or ALCOHOLIC BEVERAGES will be allowed in any prom car by ANYONE. THERE'S ABSOLUTELY NO SMOKING.
   No weapons of any kind will be permitted in our limos. Each student, if asked, must prove that he or she is not carrying a concealed weapon.

PARTITION WILL NOT BE CLOSED
In the event that a member of the party violates any of the conditions of this contract, the service will be terminated immediately and all monies will be forfeited. These conditions are designed so as to ensure that everyone will have a safe and wonderful time.
